From Hospital To Home: Moving Through The Health Care System
When an individual is ill, their first stop is the Acute Care Facility (Hospital). Acute Care is the most expensive form of care. The objective is to move the patient as quickly as possible to the next lowest cost care environment. Sometimes this is a Rehabilitation Facility, a Sub-Acute Care Facility (Nursing Home) or, optimally, the Home Care environment. The transition from one environment to the other is facilitated by professional tasked to provide the necessities of that transition. The Case Managers, Discharge Planners, and Physical and Occupational Therapists recommend the specific equipment and services required by the patient.
Typically, when a patient is discharged to the Home Care environment they are initially provided visitation care by a Home Health Care Agency. The duration and intensity of that care is determined by the type of illness. Often, a family member or loved one is left to care for the patient with little or no support.
Patients with mobility issues can present an injury risk to their caregivers, but without the ability of ambulating and transferring the patient, their general well-being is compromised. This can inevitably lead to re-hospitalization or, at the very least, a deterioration of quality of life.
